BEFORE YOUR VISIT
Before your appointment with the Ann Arbor Center for Developmental and Behavioral Pediatrics, you must complete one of the following forms and mail it to the Center at:

Here are some other helpful things you should know before your visit to the Ann Arbor Center for Developmental and Behavioral Pediatrics.
- We do not bill insurance companies. Families receive coded receipts to submit for reimbursement.
- A referral from your primary care physician is recommended.
- Exam rooms have lots of toys and we make the experience safe and fun for the children. No shots given or blood taken.
- If it is necessary to cancel your appointment, we ask you notify us at least 48 hours (two days) in advance. This courtesy allows other patients who are waiting for an appointment to use this time slot. For cancellations after that time, there will be a $50 charge. For no call/no show appointments, there will be a charge equal to half the cost of the visit.
- Please arrive 15 minutes early and call at least 2 days before if you must cancel your appointment.
- Please mail or bring as much information on your child as possible (evaluations, reports and test scores). Families will also fill out a standard medical history form and other forms.
For Dr. Solomon:
- New visits last from one to one and a half hours. Return visits last from 20 minutes to one hour based on follow-up required.
- Visits include history taking, physical exam and developmental evaluation (usually play based and observational).
- Reports are sent to the family and primary physician (unless requested otherwise by the parent).
- Audiotaping of recommendations is frequently provided.
For Dr. Bowers:
- New visits last one hour. Therapy sessions last 50 minutes and psychological evaluations are scheduled on an individual basis.
- Visits include history taking and developmental evaluation (usually play based and observational).
